ModivCare provides non-emergency transportation to and from medical service appointments.  The Division of Medicaid and Medicare Assistance contracts for these services with ModivCare.

All fully eligible Medicaid clients are eligible for non-emergency transportation.

Transportation arrangements should be made no less than two days in advance of a scheduled appointment. Modivcare will arrange for appropriate transportation to the covered medical service by one of their contracted transportation providers.
